Well, religion is a mental illness.
In any other context:
1) Talking to someone who isn't there...
2) Hearing or believing you hear someone who isn't there...
3) unusual thoughts or perceptions, including hallucinations, delusions, thought disorder, and disorders of movement (god moving the universe with you in mind, basically)
