I would keep the questions simple and to the point. One asking if they accept evolution as the theory that defines how life evolved on earth. A second asking if they believe global climate change is happening and man made. Not as sure about the third one. I'm tempted to say something generic such as what they think the role of science should be in the economy or education but its too easy to waffle out of those types of questions without really answering.
