This question is much more pressing in diseases like sickle cell disease or cystic fibrosis in which the genetics completely determine whether the disease is expressed. In my opinion anybody who has a 25 % chance of having a child with such a disease should not even try and take the chance. Maybe it helps to imagine how you justify your choice to your 16 year old sick child. Please use embryo selection in such a case.
